Not to sound like a Pollyanna but I'm curious. I debarb all my hooks and after today I'll be filing the barb down more than I have previously.

I was on the river today having a blast and taking rainbow after rainbow on the swing when one attacked my line right after my fly landed. She ran with it hard and gave me a good go. When I went to release the hook some how was caught in the roof of the mouth and eventually came free but the fish didn't make it despite being in the water. It has only happened once that I recall before but it's upsetting. Anyway, take a look at your hooks and I would recommend filing the barb not just crushing it

This will happen occasionally even with debarred hooks.

Had that happen with a steelhead, talked to a good guide buddy and he has seen it happen on occasion. What happens when hooked sometimes in the roof of the mouth you get brain penatration with the hook point, first thing you'll notice is that the eyes of the fish will look odd.
