Take the fly

Anyone know how a steelhead takes a fly? I'm wondering about hook location and fly length. I've seen a couple underwater videos but it's not with a fly. If I was a fish I might aim for the middle - less chance of missing. In most cases the fly runs parallel to the flow unless you have a big belly going in which case she'd hit it broadside? Most spey flies I see have the hook at the rear - nowhere near the head like many salt flies. The hook really extends beyond the feathers on traditional spey and Dee flies. West coast intruder style flies are even larger/longer with the hook at the rear. All of my fish landed have the hook in the mouth and jaw but I never see the ones that fail to hook up or get off. Be intersted in any thoughts.
